Application Guidelines
1. A bank must be headquartered in one of the following states: Alaska, Arizona, California, Colorado, Hawaii, Idaho, Montana, Nevada, New Mexico, Oregon, Utah, Washington or Wyoming.
2. Entries must be made by a bank’s primary location; branch locations may not enter.
3. A bank is allowed to enter both the Green Leaf Award and the Community First Award.
4. The specific achievement(s) described in the entry must have occurred between July 1, 2009, and June 30, 2010 — although they may be ongoing.
5. A bank must complete the entry form, including additional information requested in the form.
6. Suppliers to the banking industry may submit entries on behalf of their clients, provided that all of the above requirements are met.
DEADLINE FOR ENTRIES: AUGUST 15, 2010
